Determinants of Length of Stay, Recurrence, and Readmission in Diabetic-Ketoacidosis

This retrospective cross-sectional study analyzed 922 patients admitted with diabetic ketoacidosis (DKA), including both type 1 and type 2 diabetes mellitus. The authors examined factors associated with three key outcomes: length of stay (LOS), DKA recurrence at 6 and 12 months, and 90-day readmission.<br /><br />Using univariate and multivariate logistic regression, the study identified several predictors. For longer hospital stay, important factors included DKA duration, urea, and the Charlson Comorbidity Index, as well as laboratory values such as sodium, hemoglobin, and potassium. For DKA recurrence, type 1 diabetes, female gender, and lack of diabetes education were significant predictors. Six-month recurrence was specifically highlighted as an outcome of interest. For readmission, the study found that 15.1% of patients were readmitted, with new diabetes in males also noted as a factor in the analysis.<br /><br />Overall, the study showed that multiple clinical and demographic factors can predict LOS and DKA recurrence after an index DKA admission in a diverse patient population. The findings suggest that identifying high-risk patients early may help clinicians target education, follow-up, and discharge planning to reduce hospital stay, recurrence, and readmissions. The authors conclude that this information can support healthcare providers and systems in improving DKA management and reducing resource utilization.
