Creation of a Feedback Culture in an Academic Medicine Hospital Medicine Division: Development, Implementation, and Findings

This project from The Ohio State University Division of Hospital Medicine developed and implemented a standardized peer feedback system to improve clinical care, reduce practice variability, and strengthen shared professional standards. The need arose after rapid division growth from 2018–2024 and a 2023 provider survey showing dissatisfaction with handoff quality during care transitions.<br /><br />Using a DMAIC quality improvement framework, the team created a novel feedback tool that could be accessed through email, SharePoint, and the electronic medical record. The rollout happened in phases: general hospitalists in August 2024, cancer medicine hospitalists in October 2024, and advanced practice providers in November 2025. Weekly reminders encouraged participation. To maintain psychological safety, submissions were reviewed weekly by staff, with urgent concerns escalated to leadership and routine feedback compiled and delivered every three months.<br /><br />By March 1, 2026, 180 hospitalists and 49 APPs had been educated on the tool. From August 2024 to February 2026, the system averaged 4.5 responses per week. In FY25, there were 200 responses from 56 individuals (34.8%), covering 103 colleagues (64.0%). In the first four months of FY26, there were 71 responses from 28 individuals (17.0%), covering 54 colleagues (32.7%). Provider satisfaction with the feedback process improved substantially after implementation and remained stable over time.<br /><br />The authors conclude that standardized feedback can promote reflection on clinical practice and improve satisfaction with the feedback process. However, sustained success depends on both workflow redesign and broader cultural change. Future plans include continued monitoring and using AI-supported tools to screen submissions, identify urgent concerns, categorize feedback, and summarize comments while preserving anonymity.
