Covid Vaccination Status and in-Hospital Outcomes in Patients With Dka

This retrospective matched cohort study evaluated whether COVID-19 vaccination improved in-hospital outcomes among 94 patients admitted with diabetic ketoacidosis (DKA) and confirmed COVID-19 infection across four Emory Healthcare hospitals from January 2021 to March 2022. Patients were matched 1:1 by vaccination status, age, sex, race, ethnicity, comorbidities, and admission labs. The main outcome was in-hospital mortality, with secondary outcomes including length of stay, insulin needs, hypoglycemia, acute respiratory failure, thrombosis, and pneumonia.<br /><br />Among the matched groups (47 vaccinated and 47 unvaccinated), baseline characteristics were similar. Mortality was 19.1% in vaccinated patients and 21.3% in unvaccinated patients, showing no significant difference. Length of stay was also similar, with a median of 7 days in the vaccinated group versus 9 days in the unvaccinated group. Rates of hypoglycemia were nearly identical (59.6% vs 57.4%). Other complications, including acute respiratory failure, thrombosis, and pneumonia, did not differ significantly between groups. The only notable difference was chronic kidney disease, which was more common in the vaccinated group.<br /><br />Overall, the study did not find evidence that COVID-19 vaccination was associated with improved short-term hospital outcomes in patients admitted with DKA and COVID-19. The authors suggest that waning immunity, reduced vaccine effectiveness against Omicron, and incomplete vaccination documentation may have influenced the findings. They conclude that more research is needed to better understand vaccine effects in patients with DKA and concurrent COVID-19 infection.
