Covid-19 and Pulmonary Embolism Outcomes Among Us Hospitalized Patients

This study examined hospitalized U.S. patients with COVID-19 in 2020 to determine how pulmonary embolism (PE) affected outcomes. Using the National Inpatient Sample database, the authors analyzed 1,659,040 COVID-19 hospitalizations and performed propensity matching to compare patients with PE to those without PE.<br /><br />The findings show that PE was associated with significantly worse in-hospital outcomes. Compared with matched COVID-19 patients without PE, those with PE had higher mortality (19.1% vs. 13.9%; adjusted OR 1.3), longer hospital stays (10.8 vs. 7.9 days), and greater need for mechanical ventilation (25.7% vs. 15.6%). They also had more vasopressor dependence, more cardiogenic shock, greater use of mechanical circulatory support, and a higher rate of sudden cardiac death.<br /><br />The study also found that male sex and older age predicted higher mortality among COVID-19 patients with PE.<br /><br />Overall, the authors conclude that pulmonary embolism substantially worsens prognosis in hospitalized COVID-19 patients. They argue for more aggressive management, earlier diagnosis, and timely treatment of PE to reduce complications, deaths, and healthcare costs during COVID-19 hospitalizations.
