Covid-19 and Dementia in the Us: In-Hospital Outcomes

This retrospective cohort study examined whether dementia was associated with worse outcomes among older adults hospitalized with COVID-19 in the U.S. Using the 2020 National Inpatient Sample, the researchers analyzed 816,960 patients aged 65 and older, comparing 180,845 patients with dementia to 636,115 without dementia. Multivariate regression and propensity score matching were used to assess in-hospital mortality and complications.<br /><br />The study found that patients with dementia had a higher risk of in-hospital death than those without dementia: 23.1% versus 18.6%, with an adjusted odds ratio of 1.2. The authors concluded that dementia is associated with increased mortality in hospitalized COVID-19 patients.<br /><br />In addition to mortality, the analysis identified several predictors of death within the dementia group, including male sex, vasopressor use, and age 75 or older. The study also reported that COVID-19 patients with dementia were more likely to develop serious complications such as myocardial infarction, acute kidney injury, acute liver failure, sudden cardiac arrest, and need for mechanical ventilation. Interestingly, they also had lower rates of some complications, including liver failure and sudden cardiac arrest in some analyses, along with longer hospital stays and lower total hospital charges.<br /><br />The authors noted important limitations: the study was retrospective, relied on previously collected administrative data, and used ICD-10 codes, which may be imperfect. There was also potential residual confounding and limited clinical detail, so the findings may not apply to outpatient settings.<br /><br />Overall, the study suggests that dementia is associated with poorer in-hospital outcomes in older adults hospitalized with COVID-19, especially higher mortality.
