Covid-19 Associated Myocarditis Clinical Outcomes Among Hospitalized Us Patients

This study examined hospitalized COVID-19 patients in the United States during the early pandemic to assess the impact of myocarditis on outcomes. Using the National Inpatient Sample, researchers analyzed 1,659,040 patients, including 6,455 with myocarditis and 1,652,585 without.<br /><br />The main finding was that COVID-19 patients with myocarditis had much worse in-hospital outcomes than those without myocarditis. Mortality was significantly higher in the myocarditis group (30.5% vs. 13.1%), with an adjusted odds ratio of 3.0. The excess mortality was especially pronounced among younger patients.<br /><br />Myocarditis was also associated with more severe complications and greater need for intensive support. Patients with myocarditis had higher rates of sudden cardiac arrest (9.1% vs. 1.8%) and cardiogenic shock (10.5% vs. 0.6%). They were more likely to require mechanical ventilation (41.7% vs. 15.8%), vasopressors (9.9% vs. 3.9%), and hemodialysis for acute kidney injury (9.5% vs. 2.6%). However, they did not require significantly more mechanical circulatory support.<br /><br />In addition to worse clinical outcomes, myocarditis was linked to higher hospitalization costs and longer or more intensive care needs overall.<br /><br />The authors conclude that myocarditis in hospitalized COVID-19 patients is associated with substantially increased morbidity and mortality. They emphasize the need for further research into prevention and treatment strategies, and suggest that vaccination may help reduce the incidence of COVID-19-related myocarditis.
