County Health and Chronic Disease Outcomes in New York State

This study examined whether county-level health disparities in New York State are linked to chronic disease outcomes. Using 2017–2019 hospitalization and mortality data for heart disease, diabetes, and cancer, the authors compared these outcomes with 2021 County Health Rankings (CHR) z-scores for 62 counties. Negative binomial regression was used to assess associations.<br /><br />Key findings showed that counties with worse health rankings generally had higher rates of chronic disease hospitalization and mortality. Higher overall health factor z-scores were significantly associated with worse outcomes for heart disease hospitalization and mortality, diabetes hospitalization and mortality, and cancer mortality. Similar patterns were seen for health behaviors, clinical care, and socioeconomic factors, which were also linked to higher chronic disease burden. Physical environment was associated with some outcomes as well, though not consistently across all diseases.<br /><br />The study also reported statewide age-adjusted rates: heart disease hospitalization was 84.2 per 10,000 people, diabetes hospitalization was 18.9 per 10,000, and mortality rates were 169.4 per 100,000 for heart disease, 17.6 for diabetes, and 137.4 for cancer. Among all counties, Bronx had the least favorable overall health factor score, while Nassau had the most favorable.<br /><br />Overall, the findings suggest that county-level health disparities are strongly associated with chronic disease burden in New York State. The authors conclude that public health efforts focused on improving health equity may be an effective way to reduce hospitalization and mortality from chronic diseases.
