Complications and Outcomes in Patients With Diabetic Ketoacidosis and Covid-19

This retrospective study examined electronic medical records from March 1, 2020 to September 21, 2021 in adults aged 18 and older hospitalized with diabetic ketoacidosis (DKA). The goal was to compare clinical features and outcomes in patients with DKA who had confirmed COVID-19 infection versus those without COVID-19.<br /><br />Among 1,940 DKA admissions, 226 patients (11.6%) had COVID-19 and 1,714 (88.4%) did not. Baseline demographics such as age, sex, and race were similar between groups. However, patients with COVID-19 presented with somewhat less severe metabolic derangement, including lower admission glucose and lactate levels and slightly higher bicarbonate.<br /><br />Despite milder initial lab findings, the COVID-19 group had worse hospital outcomes. They took longer to reach glucose normalization, had much higher mortality (15.9% vs 2.8%), longer hospital stays, and longer ICU stays. They also experienced more complications, including acute respiratory failure, need for mechanical ventilation, sepsis, thrombotic events, and hypoglycemia during hospitalization. Severe hypoglycemia was also more common in the COVID-19 group.<br /><br />When outcomes were compared across four COVID-19 waves, mortality remained high throughout the pandemic, and differences between waves were not statistically significant, though there was a non-significant downward trend over time.<br /><br />Overall, the study found that COVID-19 infection in patients hospitalized with DKA was associated with substantially worse outcomes and higher mortality, even though patients initially appeared to have less severe hyperglycemia and acidosis. The authors conclude that further research is needed to understand how evolving COVID-19 treatments may affect DKA outcomes.
