Comparison of Resident-Led Internal Medicine Morning Report Learning Objectives With American Board of Internal Medicine Learning Objectives

This study examined whether resident-led Internal Medicine Morning Report (MR) learning objectives align with the American Board of Internal Medicine (ABIM) exam blueprint, and whether “zebra” cases become more common later in the academic year.<br /><br />Researchers reviewed morning report slides from three academic years (2018–2021) at a single residency program. They extracted Morning Report Learning Objectives (MRLOs) from titles and final slides, matched them to ABIM content and cross-content categories, and compared observed versus expected proportions using chi-square tests. They also identified uncommon diagnoses (“zebras”), defined as diagnoses not included in the ABIM blueprint, and assessed whether their frequency changed across the year.<br /><br />A total of 458 morning reports were reviewed, with an average of 4.2 MRLOs per report. Overall, the content distribution of MRLOs matched the expected ABIM content breakdown, suggesting the clinical topics covered were broadly aligned with certification priorities. However, the observed distribution differed significantly from expected proportions in cross-content categories, indicating some imbalance in topics such as ethics, prevention, geriatrics, safety, and women’s health.<br /><br />The study also found that zebra diagnoses increased significantly in the final third of the academic year. The incidence of zebras by tertile was statistically significant (chi-square 6.5, p=0.04), suggesting that more uncommon cases are selected later in the year.<br /><br />The authors conclude that resident-led morning reports generally reflect ABIM-relevant content, but their cross-content emphasis is uneven. They also note a higher frequency of uncommon diagnoses late in the academic year. Further research is needed to understand what drives case selection and how morning report can be optimized as a teaching tool.
