Comparing the Impact of Comorbidities in Patients Discharged With and Without Anemia Following an Acute Care Hospitalization

This retrospective chart review examined whether anemia at hospital discharge is associated with greater healthcare utilization (HU) over 12 months, and how comorbidities affect that relationship. The study included 2,410 patients discharged from Lincoln Medical and Mental Health Center between January 1 and May 15, 2022. After excluding patients with severe anemia, active cancer, and short stays under 24 hours, 1,652 patients remained: 870 without anemia, 437 with mild anemia (hemoglobin 10–11.9 g/dL), and 345 with moderate anemia (hemoglobin 7–9.9 g/dL).<br /><br />The main outcome was combined emergency room and inpatient visits over 12 months. ANOVA showed significantly higher HU in both anemia groups compared with controls, with the greatest utilization in the moderate anemia group. At 12 months, mean visits were 1.325 for moderate anemia, 1.124 for mild anemia, and 0.771 for no anemia. <br /><br />The study also assessed the role of chronic comorbidity burden using the Charlson Comorbidity Index (CCI). In the mild anemia group, patients with high CCI had significantly more visits than those with low CCI. In the moderate anemia group, the pattern was reversed, with low CCI patients showing more visits than high CCI patients.<br /><br />A chi-square analysis found that several comorbidities were significantly associated with 12-month healthcare utilization, including hypertension, coronary artery disease, congestive heart failure, peripheral vascular disease, CKD, ESRD, cirrhosis, peptic ulcer disease, cancer, and HIV. CKD and ESRD showed the strongest associations.<br /><br />Overall, the study concludes that mild and moderate anemia are linked to increased healthcare utilization compared with no anemia, and that chronic comorbidities, especially kidney disease, play an important role in this increased use of care.
