Comparing Picc Placement by Vascular Access Specialists Versus Interventional Radiologists

This study compared PICCs placed by vascular access nurses versus interventional radiologists (IR) across 13 Michigan hospitals from January 2013 to November 2022. The goal was to describe differences in patient and device characteristics between the two provider groups so that future studies can better evaluate complication risk.<br /><br />Among 25,276 PICCs, 71.1% were placed by vascular access nurses and 28.9% by IR. The study found meaningful differences in both patient and catheter characteristics. PICCs placed by IR were more often used in patients with renal failure, paralysis, and a history of breast cancer, and in patients who already had another central venous catheter in place. IR-placed PICCs were also more likely to be single-lumen, removed within 5 days, and placed in patients with reduced kidney function (eGFR 45).<br /><br />Because these patient and device factors are known to affect PICC-related outcomes, the authors caution that simple comparisons of complications between provider types could be misleading. They conclude that more rigorous methods, such as target-trial emulation, are needed to determine the true relationship between catheter outcomes and whether PICCs are placed by vascular access nurses or IR providers.<br /><br />The study was supported by Blue Cross Blue Shield of Michigan and Blue Care Network.
