Comparing Cardiovascular Outcomes Among the Masld Population: The Role of Metabolic Comorbidities in Patients With Low and Moderate Fibrosis-4 Index

This study examined whether metabolic comorbidities increase cardiovascular risk in patients with metabolic dysfunction-associated steatotic liver disease (MASLD) who have low or moderate FIB-4 scores. Using electronic health record data from Lincoln Medical Center (Bronx, NY) collected between January 2023 and March 2024, researchers analyzed ambulatory primary care patients and excluded those with alcohol use disorder or insulin therapy.<br /><br />Patients were grouped by FIB-4 score into low-risk (FIB-4 < 1.3; n = 9,344) and moderate-risk (FIB-4 ≥ 1.3; n = 4,442). The main cardiovascular outcomes were congestive heart failure (CHF), coronary artery disease (CAD), and peripheral artery disease (PAD). Metabolic comorbidities evaluated included hypertension, diabetes, hyperlipidemia, and obesity. Logistic regression analysis was performed in RStudio.<br /><br />The findings showed that hypertension, diabetes, and hyperlipidemia were significant risk factors for worse cardiovascular outcomes in both low- and moderate-FIB-4 groups. Obesity was also assessed, though the main highlighted associations were with the other three metabolic conditions.<br /><br />The study concludes that FIB-4 may be useful not only for liver fibrosis assessment but also as a potential tool for cardiovascular risk stratification in MASLD patients. Identifying patients with low or moderate FIB-4 scores who also have metabolic comorbidities may help flag those at higher risk for CAD, CHF, or PAD. The authors note that additional prospective studies are needed to better control for confounding factors and confirm these results.
