Common Concerns of Early-Career Hospitalists During Clinical Coaching Encounters

This study examined the main concerns of early-career hospitalists during a year-long clinical coaching program at a single academic tertiary care center.<br /><br />Background: The transition from resident to hospitalist can affect well-being and self-directed learning, so the program aimed to identify what new faculty needed most from coaching.<br /><br />Methods: All first-year hospitalists (“coachees”) were assigned a coach from November 2021 to October 2022. Coaches, who were mid- to senior-level faculty, met regularly with coachees during clinical blocks and every four months for development discussions. They recorded each coaching encounter in REDCap using 22 clinical themes plus an “Other” category and added brief free-text notes. Data were analyzed with descriptive statistics and content analysis.<br /><br />Results: Twenty-five new faculty and five coaches participated. Coaches logged 239 coaching encounters. The most common primary theme was diagnostic accuracy, including managing diagnostic uncertainty, accounting for 52 entries (21.8%). The next most frequent category was “Other” (38, 15.9%), which often included career advice, teaching methods, work/life balance, and resident leadership skills. Other notable themes were clinical management change (8.0%), assessing readiness for discharge (6.3%), and emotional support/strategies (5.9%). Example comments showed concerns about unclear diagnoses, deciding next diagnostic steps, and avoiding premature anchoring. Non-clinical concerns also included self-advocacy within the division and improving teaching strategies.<br /><br />Conclusion: Diagnostic accuracy was the leading clinical concern among new hospitalists, but they also sought support in important non-clinical areas such as career development and teaching. These findings can help hospital medicine programs tailor faculty development and refine clinical coaching for early-career hospitalists.
