Closing the Gap in RSV Prevention: Increasing Nirsevimab Uptake Among Infants

This project evaluated efforts to improve RSV prevention by increasing nirsevimab uptake among eligible infants during the 2024–2025 RSV season. RSV is a major cause of infant illness, and nirsevimab, approved in 2023, offers effective protection against RSV-related hospitalization. The institution aimed to raise uptake from 0% in the prior season to 45%.<br /><br />A retrospective, IRB-approved chart review was conducted for children aged 0–24 months seen at pediatric outpatient sites from October 1, 2024 to May 31, 2025. Infants with prior RSV prophylaxis, age 24 months, or maternal RSV vaccination within two weeks of delivery were excluded. Uptake rates were analyzed using chi-squared testing.<br /><br />Among 3,576 eligible patients, 1,551 received nirsevimab, for an uptake rate of 43.3%. This represented a significant increase compared with the previous season’s 0% uptake and was consistent with national trends. In a single-site subset of 585 infants, 77 had mothers who were vaccinated, indicating that overall RSV protection was higher than nirsevimab use alone suggests.<br /><br />The main barriers identified were inconsistent EMR documentation and weak linkage between orders and diagnoses. The authors concluded that institutional uptake aligned with national averages, but total RSV protection was likely underestimated because maternal vaccination was not fully captured. Future improvement efforts will focus on better documentation, continued provider and family education, and improved cross-site communication to track RSV prevention in real time for both infants and mothers.
