Clinical and Functional Characteristics of Long Length of Stay Inpatients at an Academic Medical Center

This retrospective cohort study from Mayo Clinic Florida examined adult inpatient admissions in 2022–2023 to characterize patients with long length of stay (LLOS), defined as 10 or more hospital days, and to assess the impact of a multidisciplinary LLOS team. Transplant and chemotherapy admissions were excluded. Among 51,918 reviewed admissions, 3,588 (6.9%) met the LLOS definition.<br /><br />Compared with other hospitalized patients, those with LLOS were older, more often identified as Black, and had a higher burden of illness, including more prior hospitalizations and higher Elixhauser comorbidity scores. They were more frequently admitted emergently or transferred from outside hospitals, and had greater use of higher-acuity settings such as ICU and PCU. LLOS patients were also more likely to be admitted for cardiovascular, respiratory, or infectious diagnoses and to require complex discharge-related resources such as dialysis, prolonged IV antibiotics, durable medical equipment, rehabilitation, skilled nursing, or home health services. Code status changes were also more common in this group.<br /><br />Overall hospital length of stay decreased modestly from 4.0 days in 2022 to 3.6 days in 2023, and the odds of LLOS also declined slightly. The findings suggest that LLOS patients represent a small but highly complex and resource-intensive population. Early identification and coordinated discharge planning are important, and the multidisciplinary LLOS team was associated with reduced odds of prolonged hospitalization, supporting the value of structured, team-based escalation for patients at risk of extended stays.
