Characterizing Risk Factors for Mortality From Sepsis During the Covid-19 Pandemic at an Urban Academic Center

This study examined factors associated with mortality among hospitalized sepsis patients during the COVID-19 pandemic at SSM Saint Louis University Hospital, an urban academic center. Using Vizient discharge data, the researchers reviewed adults aged 18 and older hospitalized between April and June 2020 with ICD-10 diagnoses of sepsis, including a subset also diagnosed with COVID-19.<br /><br />Among 366 sepsis patients, 71 had COVID-19. The cohort was nearly evenly split by race, with 48.6% African American and 48.4% Caucasian patients. Males made up 57.9% of admissions and females 42.1%. The average patient age was 60 years, and insurance coverage was predominantly Medicare (53%), followed by Medicaid (24%), commercial insurance (15.8%), uninsured (4.9%), and military insurance (2.2%).<br /><br />Clinical outcomes showed substantial illness severity. The average hospital length of stay was 13 days, with a median of 10 days. About 22.4% of patients died, 5.2% were discharged to hospice, 6.3% to long-term acute care, and 22.1% to acute rehab or skilled nursing facilities. A total of 227 patients required ICU care, with an average ICU stay of 9 days.<br /><br />Logistic regression was used to assess predictors of mortality. Race and age were significant contributors to the model, while COVID-19 diagnosis was not found to be a significant predictor of death.<br /><br />The study concludes that demographic factors, especially age and race, may influence sepsis outcomes during the COVID-19 era. The authors recommend further research to better understand how patient characteristics affect mortality, length of stay, and discharge outcomes in sepsis patients.
