This report examined central line-associated bloodstream infections (CLABSIs) on the Department of Medicine inpatient services at a large urban academic center over an 8-month period. CLABSIs are a major cause of illness and death, with prior evidence showing increased in-hospital mortality, longer hospital stays, and higher costs.<br /><br />The study looked at both patient-related and hospital-related factors, including adherence to a prevention “best practice” bundle. This bundle included daily chlorhexidine gluconate (CHG) bathing and switching from intravenous to oral medications when appropriate.<br /><br />The main findings were that patients who developed CLABSIs were more likely to have:<br />- a hospital length of stay greater than 30 days,<br />- use of total parenteral nutrition (TPN),<br />- pressure injuries.<br /><br />Even though the service followed the prevention bundle, the number of CLABSI cases increased. The authors suggest that patients hospitalized in the post-COVID era may be sicker and may have risk factors that differ from those seen in earlier studies.<br /><br />In summary, the study identifies prolonged hospitalization, TPN, and pressure injuries as important risk factors for CLABSIs in this setting, and it highlights that standard prevention practices may not be enough if patient acuity has changed.
