Characteristics Associated With Completion of Early Intervention Referral and Enrollment Among Infants With Neonatal Abstinence Syndrome

This study examined which infant and maternal characteristics were associated with successful early intervention (EI) referral/enrollment among infants with neonatal abstinence syndrome (NAS). The authors focused on completion of an individualized family service plan (IFSP), using it as the furthest measurable step toward EI enrollment.<br /><br />The researchers reviewed neonatal discharges from Baystate Medical Center in 2017. They identified NAS cases through chart review and administrative billing codes, then confirmed exposure type and extracted clinical and demographic factors including race, NICU hospitalization, maternal medication-assisted treatment (MAT) for opioid use disorder, and insurance/payor status. They also compared infants with opiate-associated NAS to those with NAS from non-opiate substances only.<br /><br />Of 125 identified discharges, 111 were included in the analysis after exclusions. Most infants (89) had opiate exposure, while 22 had only non-opiate exposure. Among infants with opiate-associated NAS, several characteristics were more common in those who completed an IFSP: NICU hospitalization, Black race, commercial insurance, and maternal MAT. However, none of these differences reached statistical significance. There was also no significant difference in IFSP completion between infants with opiate-associated NAS and those with non-opiate-associated NAS only.<br /><br />Overall, the findings suggest possible trends that may be linked to better EI follow-through, but the small sample size limited statistical power. The authors conclude that more fully powered studies are needed to better understand barriers and facilitators to EI enrollment for infants with NAS.
