Central Lines in the Medical-Surgical Wards: A Six-Year Overview of Placement by a Hospitalist Bedside Procedure Service

This six-year review describes a hospitalist Bedside Procedure Service (BPS) that began placing central venous catheters (CVCs), including temporary dialysis catheters, directly on medical/surgical wards at a hospital starting in 2015. Before this program, CVCs were placed only in the ICU or radiology. The goal was to assess the volume, safety, resident involvement, and failure/complication rates of ward-based CVC placement.<br /><br />All procedures were performed with direct ultrasound guidance and portable cardiac monitoring. Patients who could not tolerate the procedure without sedation were referred to the ICU or radiology. Data from 2015–2020 were reviewed from internal logs.<br /><br />Over the study period, the BPS completed 371 CVC procedures. Residents participated in 119 cases (32.1%). A total of 36 procedures (9.7%) were unsuccessful and required referral, most often because of anatomical abnormalities or inability to tolerate the procedure without sedation. Only 3 complications occurred (0.81%): one ventricular tachycardia arrest, one vasovagal bradycardia, and one pneumothorax.<br /><br />The authors conclude that hospitalists can safely and efficiently perform bedside CVC placement on medical/surgical wards in selected patients, with very low complication rates. They also suggest this model improves resource use, increases procedural efficiency, and provides valuable procedural experience for residents.
