Centenarians and Hip Fracture, What Should We Expect From Them?

This retrospective single-center study examined centenarian patients admitted with hip fracture between 2017 and 2021. Among 3,531 hip fracture cases, only 26 patients were centenarians (0.7%), including one patient admitted twice. Their median age was 101 years, with a high comorbidity burden (adjusted Charlson Comorbidity Index 6.5).<br /><br />At baseline, most were still able to walk (80.8%), but falls in the previous year were common (69.3%). Functional dependence was substantial: 30.8% were partially dependent in basic activities of daily living, 46.2% moderately dependent, and 23.1% totally dependent. Most lived at home with family (65.4%), while 30.8% resided in nursing homes. Intracapsular fractures were the most frequent type (57.7%).<br /><br />Management showed that surgery was feasible, but perioperative morbidity was very high. Common complications included delirium (92.3%), blood transfusion (50%), urinary tract infection and acute urinary retention (both 26.9%), congestive heart failure (19.2%), and respiratory infection (11.5%). Surgical complications were uncommon but present, including infection (3.8%), hip dislocation (7.7%), and reoperation (7.7%).<br /><br />Hospital stay was prolonged, with a median of 12 days. At discharge, 62.5% were able to walk with supervision. Half returned home, while 42.3% required an intermediate care facility. In-hospital mortality was 7.7%. However, post-discharge mortality remained high, reaching 30.4% at 3 months and 50% at 12 months.<br /><br />Overall, the study concludes that centenarians represent a small fraction of hip fracture patients, but they have significant frailty, high complication rates, and poor long-term survival. Nonetheless, meaningful recovery after surgery is possible, especially with careful management tailored to their pre-existing comorbidities and functional status.
