Care Quality and Outcomes of Sepsis in an Observation Unit

This retrospective cohort study examined community-acquired sepsis (CAS) in observation units (OUs) versus general inpatient units (GIUs), focusing on care quality and outcomes when sepsis onset occurred within 48 hours of admission. The study included 50 OU patients and 710 GIU patients.<br /><br />Patients in the two settings had similar demographic and comorbidity profiles, including the same median comorbidity score (10 vs. 10) and similar age. However, outcomes were better in the observation unit. OU patients had a shorter hospital length of stay (10 vs. 16 days) and lower in-hospital mortality (6% vs. 20%) and 30-day mortality (8% vs. 22%) compared with GIU patients.<br /><br />Quality of sepsis care was also better in the OU. SEP-1 bundle compliance was significantly higher in the OU (24% vs. 13%). Specific elements of the bundle, such as lactate measurement and blood cultures, were performed more often in the OU, while IV fluid and antibiotic administration were similar between units.<br /><br />After adjustment, OU care was associated with lower in-hospital mortality and significantly lower 30-day mortality. The authors concluded that observation units showed more compliant sepsis care and better outcomes than inpatient units, but further research is needed to identify the structural and practice features that explain these performance differences.
