This umbrella analysis evaluated whether SGLT-2 inhibitors reduce the risk of cardiovascular death or hospitalization for heart failure in patients with heart failure. The authors noted that while several meta-analyses already suggested benefit, it was unclear whether those reviews included all relevant primary studies.<br /><br />Nine systematic reviews/meta-analyses were included, and the authors assessed their quality using the AMSTAR 2 tool. They found overlap among the primary studies, but no single review contained all available studies. Using an inverse-variance pooled analysis in RevMan, they found that SGLT-2 inhibitors significantly reduced the composite outcome of cardiovascular death or hospitalization for heart failure compared with control. Reported reductions ranged from 21% to 30%, and the pooled estimate showed a 24% relative risk reduction overall (HR 0.76; 95% CI 0.74–0.78; p = 0.01; I² = 0%).<br /><br />The conclusion was that SGLT-2 inhibitors provide substantial benefit in heart failure, especially in reducing major hospital-related cardiovascular outcomes. The authors reported no financial disclosures.
