Can Health Confidence Help Predict Hospital Discharge Disposition?

This prospective cohort study examined whether the Health Confidence Score (HCS) could predict hospital discharge disposition, especially need for post-acute care (PAC). General medicine inpatients at the University of Chicago Medical Center completed the HCS early in hospitalization as part of a larger survey (January–October 2022). The study included 2,797 participants, with a 29% response rate. Patients were mostly female (55%), Black/African American (70%), non-Hispanic (87%), and had a mean age of 60 years. The average HCS was 9 out of 12, and 61% scored at or above the mean.<br /><br />Discharge outcomes were categorized into home without PAC, home health, skilled nursing facility, acute rehabilitation, long-term acute care hospital, hospice, and against medical advice (AMA). Using bivariate analysis and multinomial logistic regression, the authors found that mean HCS differed significantly across discharge groups. Patients discharged home or AMA had the highest health confidence, while those discharged to hospice had the lowest. Higher HCS was associated with a greater predicted probability of discharge home without PAC and a lower risk of requiring PAC compared with home discharge. AMA patients did not differ significantly from home discharges in health confidence.<br /><br />The study concludes that health confidence measured at admission may help identify patients likely to need PAC services. This could improve triage by social work and case management and support more timely discharge planning. The authors note this is the first study to examine the relationship between HCS and hospital discharge disposition.
