Calcitonin Gene-Related Peptide (Cgrp) Antagonist vs. Standard Care for the Management of Resistant Migraine: An Umbrella Meta-Analysis

This umbrella meta-analysis found that calcitonin gene-related peptide (cGRP) antagonists were more effective than standard management for resistant migraine. Overall, cGRP antagonists showed a 61% higher efficacy, with a pooled odds ratio of 1.61 (95% CI: 1.49–1.75, p = 0.00001). Heterogeneity was moderate to high overall (I² = 72%), though subgroup heterogeneity was lower (I² = 34.7%).<br /><br />In subgroup analysis, Erenumab 70 mg had the highest efficacy for achieving a 50% reduction in monthly migraine days. Its effect size was OR 1.80 (95% CI: 1.35–2.40, p = 0.0001), although heterogeneity was high (I² = 91%).<br /><br />The authors conclude that cGRP antagonists provide a clear benefit over standard care in resistant migraine, with Erenumab 70 mg appearing especially promising. They recommend further prospective studies to confirm these findings and to examine how patient factors such as age, race, sex, disease duration, and migraine type may influence treatment response.
