Calcinosis Universalis Secondary to Silicone Injections in a Patient With Hiv and Chronic Kidney Disease: A Case Report of Silicone-Induced Hypercalcemia

This case report describes a 44-year-old African American man with HIV and chronic kidney disease who presented with headache, gait imbalance, and flank pain. Laboratory testing showed moderate hypercalcemia with acute kidney injury and an inappropriately normal parathyroid hormone level, suggesting a non-parathyroid cause of hypercalcemia. CT imaging revealed bilateral hyperlucent/calcified deposits in the gluteal region, and the patient later disclosed a history of silicone injections performed about 20 years earlier.<br /><br />He was initially treated with intravenous fluids and pamidronate, which temporarily normalized his calcium levels. However, he was readmitted one month later with recurrent hypercalcemia. Because of his chronic kidney disease, nephrology recommended denosumab rather than additional bisphosphonates. After one dose, his calcium improved, though he was eventually lost to follow-up before completing treatment.<br /><br />The report emphasizes that calcinosis universalis secondary to silicone injections is a rare but important cause of hypercalcemia, especially as cosmetic procedures become more common. Clinicians should consider this diagnosis in patients with unexplained hypercalcemia and a history of cosmetic injections or foreign material exposure. Early recognition is important to guide proper treatment and prevent complications.
