This meta-analysis examined whether inhaled corticosteroid (ICS) use affects mortality in patients with COVID-19, especially among those with asthma or COPD. The authors followed PRISMA and MOOSE guidelines and searched PubMed for studies published from December 2019 to August 2021. From 270 articles, six studies were included. Outcomes were pooled using random-effects models in RevMan 5.4.<br /><br />Across the included studies, five reported mortality. The overall mortality rate among the analyzed COVID-19 patients was 7.49% (107/1428). Mortality was especially high in patients with asthma (60.88%, 107/160) and COPD (68.46%, 382/558) in the study populations examined.<br /><br />The main meta-analysis found that ICS use was associated with significantly higher odds of death in COVID-19 patients compared with standard care or non-ICS treatment (OR 1.45, 95% CI 1.10–1.91; p = 0.009), with moderate heterogeneity (I² = 68%). In subgroup analysis, the increased mortality risk remained significant among COPD patients using ICS (pooled OR 1.52, 95% CI 1.24–1.86; p = 0.0001; I² = 0%). However, no significant association between ICS use and mortality was found among asthma patients.<br /><br />The authors conclude that ICS use may be associated with increased mortality in COVID-19, particularly in COPD patients, and that more prospective studies are needed to confirm the safety and efficacy of ICS in this setting.
