Burden of Treatment in Heart Failure Follow Up Study

This study examined burden of treatment (BoT) and social determinants of health (SDoH) in patients hospitalized for heart failure (HF) exacerbations at Lincoln Medical Center from March to October 2022. Among 113 enrolled patients, 74 had HFrEF and 39 had HFpEF. HFpEF patients were older on average and were more often women. HFpEF was more associated with hypertension and obesity, while HFrEF had more CAD, OSA, and significantly higher substance use (alcohol, cocaine, heroin, tobacco).<br /><br />SDoH challenges were common in both groups, including low education, food insecurity, housing instability, and unemployment, with some measures worse in HFrEF. BoT was assessed using the Patient Experience with Treatment and Self-Management (PETS) tool across domains such as medical information, medications, appointments, health monitoring, healthcare services, and physical/mental fatigue. HFpEF patients reported higher baseline burden in nearly all BoT domains, including interference with work, family, daily activities, healthcare access, medication management, monitoring, and understanding medical information.<br /><br />At 6-month follow-up, only 20 of 42 eligible patients responded (6 HFpEF, 14 HFrEF). HFrEF patients showed improvement in most BoT domains, especially medical information, medical expenses, and difficulty with healthcare services. In contrast, HFpEF patients worsened across all domains, especially in role/social limitations and physical/mental exhaustion. No meaningful changes in SDoH were seen at follow-up.<br /><br />The study concludes that SDoH and substance use play an important role in HF disparities. HFpEF patients, particularly women, experienced a greater and worsening treatment burden without clear improvement over time, highlighting the need for targeted interventions and further follow-up.
