Building the Next Generation of Hospitalists Through Individualized Curriculum
Back to course
Pdf Summary
The document describes a redesigned hospital medicine (HM) elective for internal medicine residents at Duke University aimed at better preparing future hospitalists through individualized training.

Originally, the elective used a fixed two-week template with one week of rounding at the academic hospital and one week of standardized clinical and non-clinical activities. After residents reported widely varying interests and goals, the program shifted to a resident-centered model launched in July 2020. The new structure allows residents to choose between traditional rounding at an academic or community hospital, perioperative medical/surgical co-management rounding, and a mix of additional activities such as admitting or consultative medicine shifts, leadership shadowing, quality improvement (QI), billing/coding education, and perioperative medicine training.

Program leaders identified gaps in the residency curriculum, especially in perioperative medicine and inpatient billing/coding, and incorporated these into the new elective. From July 2020 to June 2022, the team collected post-rotation feedback from residents; later, graduates working as hospitalists were also asked to reflect on the elective’s value.

Feedback was largely positive. Residents highlighted the breadth of experiences, ability to personalize schedules, exposure to new HM roles, and direct mentorship from faculty. Some noted that the elective helped them feel more prepared for hospitalist jobs and even provided an advantage in the job market. Graduates also reported that the elective strengthened their confidence and gave them experience with unique clinical and administrative aspects of hospital medicine.

The main conclusion is that a personalized HM elective can be highly valuable for developing aspiring hospitalists, though it may be time-intensive to schedule and difficult to standardize in smaller or less-resourced settings.
