Bridging the Gap: How Sglt2 Inhibitors Are Transforming Hfpef Outcomes

This poster presents a meta-analysis evaluating whether SGLT2 inhibitors improve outcomes in heart failure, with a focus on the difference between HFrEF and HFpEF. The authors note that HFpEF remains difficult to treat because of its heterogeneous causes and limited effective therapies, despite interest in SGLT2 inhibitors such as dapagliflozin.<br /><br />A literature search of CENTRAL, Google Scholar, PubMed, CINAHL, Scopus, and Web of Science through 2022 identified randomized controlled trials comparing SGLT2 inhibitors with placebo. Data were extracted using standardized methods, and study quality was assessed with the Cochrane Risk of Bias tool. Forest plots and heterogeneity analyses were used to summarize results.<br /><br />The analysis found a clear benefit in HFrEF. Across 18,679 participants, SGLT2 inhibitors significantly reduced adverse events versus placebo, with an odds ratio of 0.73 (95% CI 0.69–0.78), representing a 27% reduction in risk. This effect was highly statistically significant, and heterogeneity was low (I² = 0%), suggesting consistent benefit across studies.<br /><br />In contrast, the HFpEF subgroup showed no statistically significant benefit. Among 5,811 participants, the pooled odds ratio was 0.99 (95% CI 0.89–1.17; p = 0.44), indicating no meaningful difference from placebo for the composite outcome. Heterogeneity was moderate (I² = 69.6%), suggesting variability among studies.<br /><br />The authors conclude that SGLT2 inhibitors appear robustly effective in HFrEF, but evidence for benefit in HFpEF remains uncertain. Possible reasons include patient heterogeneity, differences in trial design and follow-up, and limited statistical power. Larger, more consistent studies are needed to better define the role of SGLT2 inhibitors in HFpEF management.
