This study, conducted from June 2023 to March 2024, evaluated a quality improvement bundle aimed at reducing 30-day COPD readmissions during a multicenter national trial to lower unplanned COPD-related ED visits. COPD is a major cause of morbidity, mortality, and healthcare use, with substantial readmission rates and a significant portion thought to be preventable through better discharge planning and self-management support.<br /><br />The study included 167 admissions for acute COPD exacerbation among 137 patients. Most patients were discharged home (84%), while 11% went to skilled nursing facilities, 4% to hospice, and 1% were transferred. Among home discharges, 74% were successfully contacted after discharge, 20% were unreachable after two calls, 5% were readmitted before contact, 1% declined participation, and the average time to contact was 8 days.<br /><br />The intervention consisted of a three-part bundle: inpatient inhaler education by respiratory therapists, post-discharge medication reconciliation, and post-discharge COPD action plan review. Education included inhaler device identification, step-by-step technique, and supervised use. However, implementation was incomplete: only 30% of patients received the full bundle and 5% received no intervention. Notably, only one readmitted patient received the full bundle.<br /><br />Outcomes suggested improvement in readmissions. The pre-implementation COPD-specific 30-day readmission rate was 14.29%, compared with 11.86% during the implementation year. Overall, the findings suggest that a coordinated discharge care bundle may help reduce COPD-specific readmissions, though delivery reach was limited and further improvement in implementation is needed.
