Bone Marrow Biopsies: The Marriage of Oncology and a Hospitalist Bedside Procedure Service

This report describes a hospitalist Bedside Procedure Service (BPS) that began performing inpatient bone marrow biopsies (BMB) at a tertiary care center in 2016. BMB are commonly needed in oncology inpatients and are usually low-risk, but they are often performed by specialists or radiology, which can increase costs and radiation exposure. To improve efficiency, hospitalist faculty were trained by the oncology department to perform standard posterior iliac crest bone marrow biopsies using an Oncontrol Power Driver drill.<br /><br />The authors reviewed internally collected data from 2016–2020 to assess procedure volume, resident participation, complications, and failed procedures. Over five years, the BPS completed 433 BMBs. Residents participated in 64 procedures (14.8%) without any associated complications.<br /><br />Outcomes were favorable: only one complication occurred, a retroperitoneal hematoma. Thirty-seven procedures (8.5%) required referral for CT-guided biopsy, most commonly because of morbid obesity or inability of the patient to tolerate the bedside procedure without sedation. The number of unsuccessful procedures varied by year but remained relatively low overall.<br /><br />The authors conclude that hospitalists can be trained to safely and effectively perform bone marrow biopsies at the bedside with high success and very low complication rates. They argue that this model improves resource use, reduces unnecessary expense and radiation exposure, and provides trainees with meaningful procedural experience. The experience may serve as a model for other institutions interested in expanding hospitalist procedural services.
