Bias Event Reporting System for Hospital Medicine ...
Bias Event Reporting System for Hospital Medicine Division
Back to course
Pdf Summary
The document describes the development of a Bias Event Reporting System for the University of Rochester Hospital Medicine Division to address bias, microaggressions, and related workplace harms. Initially, the division relied only on the institution’s general safety reporting system (RLSolutions), which did not allow meaningful follow-up. To improve this, the team created a dedicated Bias Incident Report Form based on the institutional trainee reporting form.

They also partnered with the Office for Equity and Inclusion to develop “Encountering Bias in Medicine” (EBiM), a training program designed to help clinicians identify and respond to bias and microaggressions. The training was grounded in Theater of the Oppressed methodology and supported a “calling-in” approach to difficult conversations around bias.

A reporting and escalation roadmap was established so that reports would go to the Division Chief and the Department of Medicine Director of DEI, with escalation to leadership such as the Department Chair or nursing leadership when needed. The system launched in Fall 2022, and most reports were submitted through self-report rather than direct reporting. Leadership appreciated the tool.

Key lessons learned were that leadership buy-in is essential, support for the person receiving reports is critical, and foundational knowledge about bias and microaggressions is necessary for the system to work well. Future plans include expanding the program to the entire Department of Medicine and using reported issues to guide faculty development.

Overall, the division presents the tool as a transparent, accessible way to improve workplace wellness and respond constructively to bias events.
