Benign Prostatic Hyperplasia: A Rare Cause of Deep Vein Thrombosis

This case report describes a rare complication of benign prostatic hyperplasia (BPH): deep vein thrombosis (DVT) caused by massive urinary bladder distention compressing the inferior vena cava (IVC) and iliac veins.<br /><br />A 62-year-old man with Parkinson’s disease presented with one month of worsening left leg pain and swelling. He had no known risk factors for DVT such as immobility, travel, malignancy, prior clots, or thrombophilia. Exam showed marked swelling of the entire left leg and severe abdominal distention. Duplex ultrasound revealed an extensive DVT from the left distal femoral vein to the external iliac vein. CT angiography of the abdomen and pelvis showed a massively distended bladder with significant compression of the distal IVC and an occluding thrombus in the left common iliac, external iliac, internal iliac, and proximal thigh veins.<br /><br />On further history, the patient reported urinary hesitancy and incomplete bladder emptying for more than a year, suggesting chronic urinary retention. The distended bladder was ultimately attributed to BPH. Treatment included heparin, Foley catheter placement, and tamsulosin. Follow-up imaging showed bladder decompression and reduced IVC compression. The patient later underwent transurethral resection of the prostate (TURP).<br /><br />The report emphasizes that although urinary symptoms are common in BPH, elderly patients may underreport them unless specifically questioned. Clinicians evaluating unexplained DVT should consider urinary retention and bladder distention as rare but important causes of venous compression and stasis. Advanced imaging may be needed when suspicion is high.
