This retrospective cross-sectional study examined BEERS Criteria prescribing among 29,655 adult admissions to a large academic hospital from August 2018 to January 2020. The AGS 2019 BEERS Criteria list medications that may be inappropriate in older adults because they can increase risks such as falls, delirium, hypotension, and urinary retention.<br /><br />The study assessed how many patients received BEERS medications by age group and whether the number of BEERS medications was associated with falls and delirium. Nearly all hospitalized adults received at least one BEERS medication, including 89% of patients aged 65 and older. Prescribing patterns differed by age, with the 80+ cohort receiving fewer BEERS medications than younger groups.<br /><br />A total of 286 patients experienced falls and 3,707 screened positive for delirium. The risk of falls increased clearly as the number of BEERS medications increased. Patients who received four or more BEERS medications accounted for half of all falls and 40% of delirious patients, suggesting a strong relationship between polypharmacy and adverse outcomes.<br /><br />The authors note that the delirium association may be partly influenced by medications used to treat delirium, but likely reflects a true contribution from BEERS medications as well. They conclude that deprescribing efforts should be prioritized in both inpatient and outpatient settings.<br /><br />Future work will examine the impact of BEERS prescribing on length of stay and discharge disposition, and identify which specific BEERS medications are most strongly linked to falls and delirium.
