This project redesigned the inpatient after-visit summary (AVS) to improve discharge clarity, provider workflow, and post-discharge follow-up. The original AVS was described as repetitive, time-consuming, and difficult for both providers and patients to use effectively, contributing to poor follow-up within 7 days of discharge.<br /><br />A multidisciplinary team including case management, nursing, pharmacy, hospitalists, and IT collaborated across four hospitals using a five-phase redesign process. The team also sought feedback from the Patient and Family Advisory Council to ensure the AVS reflected patient needs. Outcomes were assessed using provider pre- and post-intervention surveys and hospital follow-up data at 7 and 14 days.<br /><br />Results showed statistically significant improvement in all provider-rated AVS domains after redesign: medication reconciliation, clarity of discharge instructions, appointment scheduling, documentation, and overall AVS process. Mean survey scores improved across the board, with the largest gains in discharge instruction clarity and documentation.<br /><br />Follow-up rates also improved. Comparing January–April to May–August, the redesign was associated with significantly better 7-day post-discharge follow-up (chi-square p = 0.001). The 14-day follow-up showed a positive but not statistically significant trend. Trend analysis across the full January–August period confirmed a significant upward trend in 7-day follow-up.<br /><br />Overall, the study suggests that a multidisciplinary, patient-informed AVS redesign can improve both provider experience and timely post-discharge follow-up, with potential implications for reducing readmissions.
