Attitudes and Practices Regarding in-Hospital Thrombophrophylaxis Risk Assessment Models

This survey studied U.S. healthcare providers’ attitudes and practices regarding venous thromboembolism (VTE) thromboprophylaxis risk assessment models (RAMs) in hospitalized patients. Because hospital-associated VTE is a major cause of morbidity and mortality, and many events occur after discharge, the authors examined how often clinicians use RAMs to guide prophylaxis and what barriers limit their use.<br /><br />A total of 372 respondents were included, most of whom were trainees (59.9%) and/or worked in academic hospitals (55.4%). Respondents generally believed in-hospital VTE prophylaxis is important, safe, and effective, but they felt institutional implementation was suboptimal. Only 56.2% thought patients at their own institutions were receiving appropriate prophylaxis almost all the time.<br /><br />Actual RAM use was modest. About 39% reported using a RAM for VTE risk assessment, and only 23% reported using a RAM for bleeding risk. Trainees were less likely than attendings to use a RAM for VTE risk assessment (adjusted RR 0.70), but more likely to use a bleeding-risk RAM (adjusted RR 1.93). RAM use did not meaningfully differ by practice setting or by interest in thrombosis/hemostasis research.<br /><br />Overall, the study suggests that while clinicians value thromboprophylaxis, RAMs are underused in routine practice. The authors conclude that improving provider education and integrating RAMs into electronic health systems may help increase consistent use and improve prophylaxis decisions for hospitalized patients.
