Atrial Fibrillation Predicts Mace in Transcatheter Tricuspid Valve Procedures

This retrospective cross-sectional study examined whether atrial fibrillation (AF) predicts adverse outcomes in patients undergoing transcatheter tricuspid valve procedures (TTVP) for tricuspid regurgitation. Using the National Inpatient Sample from January 2017 to December 2019, the authors compared hospitalized TTVP patients with and without AF. They used chi-square testing for baseline differences and multivariable logistic regression adjusted for age, sex, and race.<br /><br />The main outcome was major adverse cardiovascular events (MACE). The study found that AF was associated with significantly worse cardiovascular outcomes after TTVP. Specifically, AF increased the odds of MACE by 3.29 times and increased the odds of heart failure by 3.05 times compared with patients without AF. However, mortality was not statistically different between groups: 3.51% in non-AF patients versus 2.74% in AF patients.<br /><br />The authors conclude that AF is an independent predictor of adverse cardiovascular events, particularly heart failure, in patients undergoing TTVP. Because TTVP is a newer and less common procedure, data on long-term and in-hospital outcomes remain limited. The study suggests that future research should identify predictors of outcomes among TTVP patients with AF and potentially develop a risk score similar to those used in transcatheter aortic valve replacement populations.<br /><br />The authors also note important limitations, including the retrospective design and potential confounding factors.
