Associations Between Obesity and Peripherally Inserted Central Catheter-Related Complications

This study examined whether obesity affects complications from peripherally inserted central catheters (PICCs) in hospitalized adults. Using data from 69 Michigan hospitals collected between 2013 and 2023, the researchers analyzed 59,694 patients with PICCs and grouped them by BMI: underweight, normal weight, overweight, and obese.<br /><br />The main outcome was major PICC complications, including central line-associated bloodstream infection (CLABSI) and catheter-related venous thromboembolism (CR-VTE). Secondary outcomes included minor complications such as catheter dislodgement, occlusion, tip migration, superficial thrombosis, exit-site problems, and overall device failure. Multivariable logistic regression adjusted for patient and catheter characteristics.<br /><br />Among patients with PICCs, 25.3% were obese. Obese patients had slightly different PICC characteristics, including a median dwell time of 10 days, a somewhat higher rate of single-lumen PICCs, and fewer patients with advanced chronic kidney disease.<br /><br />The key finding was that obesity was not associated with higher odds of major complications or device failure. However, obesity was associated with increased minor complications. Specifically, obese patients had higher odds of minor PICC complications overall, especially catheter occlusion and exit-site problems.<br /><br />In summary, obesity does not appear to increase the risk of serious PICC complications, but it is linked to more minor device problems. The authors suggest this may reflect an opportunity to improve PICC maintenance practices in patients with obesity.
