Association of Abnormal Echocardiographic Diastolic Parameters and Postoperative Major Adverse Cardiac Events and Mortality in Patients Undergoing Hip Fracture Surgery

This retrospective study examined whether abnormal preoperative echocardiographic diastolic parameters predict poor outcomes after hip fracture surgery. The authors reviewed 148 patients who underwent hip fracture repair between April 2016 and June 2021 and had a recent echocardiogram. Diastolic dysfunction was assessed using three measures: E/e’ ratio, tricuspid regurgitant velocity (TRV), and left atrial volume index (LAVI). Patients were grouped by the number of abnormal parameters: 0–1 abnormal versus 2–3 abnormal.<br /><br />Patients with 2–3 abnormal diastolic parameters had more comorbidities, including ischemic heart disease, atrial fibrillation, pulmonary hypertension, and chronic kidney disease. They also experienced worse postoperative outcomes. Major adverse cardiac events (MACE) occurred in 23.9% of patients in the 2–3 abnormal group compared with 8.6% in the 0–1 group (P=0.011). One-year mortality was also higher (31.3% vs 13.6%, P=0.009), as was 2-year mortality (43.3% vs 19.8%, P=0.002).<br /><br />Overall, postoperative MACE occurred in 15.5% of patients, with 1-year mortality of 21.6% and 2-year mortality of 30.4%. The findings suggest that a greater burden of abnormal diastolic echocardiographic parameters is associated with increased risk of postoperative MACE and mortality after hip fracture surgery. The authors conclude that evaluating E/e’, TRV, and LAVI together may help identify high-risk patients and guide preoperative optimization and postoperative care.
