Association Between Sleep and Health Literacy in Diabetic Inpatients

This retrospective study examined 721 hospitalized adult patients with diabetes to assess whether subjective sleep quality is related to health literacy and whether poor sleep predicts longer hospital stay. The researchers used health literacy measured by the Brief Health Literacy Screen (BHLS), subjective sleep measures including the Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), and Sleep Self-Efficacy scale, and compared these with length of stay (LOS).<br /><br />About 21% of patients had low health literacy. Patients with low health literacy had worse PSQI scores, suggesting poorer perceived sleep quality, but health literacy was not significantly associated with daytime sleepiness (ESS) or sleep self-efficacy. Overall, subjective sleep quality was not significantly associated with LOS in the full sample. However, when results were stratified by health literacy, poor subjective sleep quality was linked to an 8.6% longer LOS among patients with low health literacy.<br /><br />The authors conclude that health literacy may influence how patients report sleep quality and may modify the relationship between sleep and hospital outcomes. Their findings suggest that low health literacy could limit the interpretation of subjective sleep assessments and may also be a risk factor for worse inpatient outcomes. The study is limited by its retrospective design, possible recall or classification bias, and unmeasured confounding. Future research should further explore how health literacy affects sleep reporting and whether improving health literacy can improve sleep-related patient outcomes.
