Association Between Lumens and Picc Complications in Cancer Patients

This retrospective cohort study examined whether the number of lumens in peripherally inserted central catheters (PICCs) affects complication risk in hospitalized adults with active malignancy. Data came from 5,609 PICC placements across 65 Michigan hospitals between 2014 and 2023. Outcomes included symptomatic upper-extremity venous thromboembolism (UE-VTE), laboratory-confirmed central line-associated bloodstream infection (CLABSI), catheter occlusion, and a composite of all three, assessed through chart review and 30-day follow-up.<br /><br />Most PICCs were double lumen (63.3%), followed by single lumen (28.7%) and triple lumen (8.0%). Complication rates increased with more lumens. Compared with single-lumen PICCs, double-lumen PICCs had significantly higher odds of UE-VTE, CLABSI, catheter occlusion, and the composite outcome. Compared with double-lumen PICCs, triple-lumen PICCs had significantly higher odds of CLABSI, catheter occlusion, and the composite outcome, but not UE-VTE.<br /><br />Key adjusted odds ratios showed:<br />- Double vs. single: UE-VTE 2.13, CLABSI 1.75, occlusion 4.00, composite 2.86<br />- Triple vs. double: UE-VTE 1.31, CLABSI 1.93, occlusion 1.55, composite 1.77<br /><br />Overall, the study found that more PICC lumens were associated with greater complication risk in patients with active cancer. The authors conclude that, when possible, single-lumen PICCs should be preferred over multi-lumen catheters in this high-risk population.
