Anticipatory Guidance and Patient Self-Management at Discharge: A Provider Survey

This survey examined how hospital-based internal medicine providers use anticipatory guidance and patient self-management during discharge planning. The authors note that clear written discharge instructions are an important tool for safe transitions of care and may help prevent readmissions.<br /><br />An anonymous web-based survey was emailed to resident physicians, attending physicians, and advanced practice providers at Cleveland Clinic Main Campus. The survey assessed providers’ knowledge, attitudes, and self-reported practices regarding anticipatory guidance and patient self-management at discharge. Eighty-seven providers responded.<br /><br />Key findings were that most respondents were familiar with both concepts and all surveyed providers believed the information is valuable for patients. However, only a minority reported consistently including these concepts in discharge instructions. This suggests a gap between awareness and actual practice.<br /><br />The authors conclude that there is a significant opportunity to improve discharge communication and patient education. Importantly, the issue does not appear to stem from lack of knowledge or awareness, and it is not limited to trainees. Future quality improvement efforts should focus on helping providers more reliably incorporate anticipatory guidance and self-management strategies into discharge instructions.
