Analysis of Geographic Rounding Affecting Observed to Expected Discharge Ratio

This study evaluated the impact of geographic rounding at two Integris Health hospitals: Baptist Medical Center and Southwest Medical Center. Geographic rounding assigns hospitalist teams to specific inpatient units to improve communication, coordination, and care efficiency.<br /><br />Using a before-and-after qualitative analysis, the study compared key metrics: Length of Stay (LOS), Observed-to-Expected (O/E) ratio, and Opportunity days. Statistical testing showed significant improvements after geographic rounding was introduced.<br /><br />At Integris Health Baptist Medical Center, LOS decreased by 0.86 days, O/E ratio decreased by 0.1, and Opportunity days decreased by 0.7 days, all statistically significant. At Integris Health Southwest Medical Center, LOS decreased by 1.1 days, O/E ratio decreased by 0.2, and Opportunity days decreased by 1 day, also with strong statistical significance.<br /><br />The findings suggest that geographic rounding improved operational efficiency by streamlining patient care processes and enhancing provider communication. Shorter hospital stays and lower O/E ratios may also indicate better resource utilization and potential cost savings. The authors note that, despite implementation challenges such as logistical changes and cultural adaptation, the benefits make geographic rounding a promising strategy for improving hospital performance.<br /><br />The conclusion emphasizes that geographic rounding can reduce hospital stay durations, improve efficiency metrics, and potentially benefit both patients and healthcare systems. The study recommends broader implementation and further research, especially on patient satisfaction, provider experience, and detailed cost outcomes.
