An Unusual Cause of Hypereosinophilia After Ascending Aortic Aneurysm Repair

This case report describes a 73-year-old woman who developed hypereosinophilia after ascending aortic aneurysm repair with hemi-arch replacement. One month after surgery, she was evaluated for low blood pressure and found incidentally to have an elevated absolute eosinophil count (AEC) of 1,800/µL, along with thrombocytosis and mild leukocytosis. Her eosinophil count continued to rise, peaking at 4,500/µL.<br /><br />A broad evaluation for common causes of hypereosinophilia—including medication reaction, travel-related infection, hematologic disease, rheumatologic disease, and other systemic illness—was unrevealing. She had no symptoms or evidence of end-organ damage, and her eosinophilia eventually resolved spontaneously.<br /><br />After the initial workup failed to identify a cause, clinicians revisited her recent surgery. During the operation, she had received TachoSil, a topical fibrin sealant patch. Post-marketing data have linked TachoSil to eosinophilia, and the timing of exposure matched the patient’s laboratory abnormalities. TachoSil was therefore determined to be the likely cause of her hypereosinophilia.<br /><br />The report emphasizes that hypereosinophilia, defined as an AEC of at least 1,500/µL, can lead to serious tissue infiltration and organ damage even when asymptomatic. It also highlights the importance of considering surgical grafts and implants, not just medications and supplements, in the differential diagnosis of unexplained eosinophilia.
