An Innovative Approach to Increase Academic Appointments for Hospital Medicine Nurse Practitioners and Physician Assistants

This document describes a quality improvement project at Mayo Clinic’s Hospital Internal Medicine division aimed at increasing academic appointments (AA) among nurse practitioners and physician assistants (NPPAs). Academic appointments—Instructor, Assistant Professor, Associate Professor, and Professor—are used to recognize scholarly and academic contributions, but NPPAs historically receive them infrequently.<br /><br />The project began because there was little published research on AA for NPPAs and only 17.5% of HIM NPPAs in Rochester held an AA in 2017. A pre-intervention survey of 38 NPPAs found that many viewed AA as important, but barriers included lack of mentorship, lack of leadership emphasis, and limited time. Based on survey findings, the team implemented targeted interventions: improved access to RE-AIMS self-editing, an informational brochure, a presentation at a monthly NPPA meeting, inclusion of an NPPA on the Division’s Faculty Development Committee, increased leadership emphasis, and mentorship support.<br /><br />A post-intervention survey of 46 NPPAs showed improved awareness and perception of AA. More NPPAs recognized AA as important, fewer reported not knowing what it was, and perceived barriers decreased, especially around mentorship and leadership emphasis. The project also showed an increase in the number of NPPAs with academic appointments over time, suggesting the approach was effective and sustainable over five years.<br /><br />The authors conclude that structured interventions can significantly improve AA attainment among NPPAs. They also note that more research is needed to determine whether this approach reduces burnout and whether it can be applied successfully at other academic institutions.
