An Assessment of Health-Related Social Needs Among Hospitalized Adults With Congenital Heart Disease

This ongoing cross-sectional pilot study at Texas Children’s Hospital examined hospitalized adults with congenital heart disease (ACHD) to assess unmet health-related social needs and mental health conditions. Adults admitted between June 2021 and March 2022 were screened; patients speaking a primary language other than English or Spanish and pregnant patients were excluded, though patients without independent decision-making capacity were later included after an IRB amendment.<br /><br />Among 67 enrolled patients, the median age was 29 years, 63% were male, and 55% had great-complexity cardiac lesions while 45% had moderate-complexity lesions. The cohort was racially and socioeconomically diverse, with notable variation in insurance, household composition, and income.<br /><br />The main finding was a high burden of unmet social needs: 66% of participants reported needs in three health-related social domains. The most common needs were stress (78%), physical activity limitations (70%), cognitive disability (40%), financial strain (36%), and physical disability (24%). Mental health symptoms were also frequent, with 64% screening positive for depression symptoms and 48% for anxiety symptoms. In addition, 28% of participants had been rehospitalized within one month of enrollment.<br /><br />The study concludes that hospitalized ACHD patients have substantial unmet health-related social needs and significant mental health burden. Because admissions for acute illness, chronic disease complications, and procedures are common in this population, hospital medicine teams have an important opportunity to identify and address these needs during hospitalization.
