Advancing Resident Near-Peer Feedback Skills Through Medical Simulation

This project addressed a common gap in medical education: residents are expected to give critical feedback to peers, but many programs do not formally teach this skill. Because intern-resident relationships can make feedback conversations uncomfortable, the team developed a simulation-based curriculum to help residents practice delivering direct, respectful, in-person feedback in a near-peer setting.<br /><br />The curriculum was created after a focus group of internal medicine residents identified training needs. A working group of senior residents, simulation faculty, and simulation center leadership designed the scenario, piloted it with sample learners, and revised it based on feedback. The simulation had two main phases: first, resident-to-resident feedback during an acute medical scenario; second, a resident-led debrief using the advocacy-inquiry framework. Participants were given limited pre-session information, including the severity of the patient’s illness and the emotional intensity of the intern’s objection.<br /><br />Six learners completed pilot sessions and provided survey feedback on confidence and perceptions. Preliminary results were encouraging: all participants reported they had rarely or never received training in feedback delivery before the simulation, and all agreed they felt less anxious about giving feedback afterward. Learners said they left with a usable structure for delivering feedback and felt more prepared to teach others the skill.<br /><br />The team concluded that the resident-led simulation was effective for teaching critical feedback delivery and improving confidence. They noted that fidelity could be improved by clarifying facilitator roles, such as distinguishing between the intern and bedside nurse. Next steps include making the simulation required for all PGY2 residents and collecting pre- and post-survey data from additional participants to further evaluate its impact.
