Advancing Perioperative Medicine Education: Ms4 Elective: Perioperative and Consultative Medicine
Back to course
Pdf Summary
The Farber Hospitalist Service at Thomas Jefferson University Hospital developed MED492, a 2-week, pass/fail inpatient elective for fourth-year medical students to teach perioperative and consultative medicine, with emphasis on neurosurgical and orthopedic spine co-management. The elective, implemented in 2020 across two sites, uses one-on-one attending preceptorship and a checklist of high-yield perioperative topics to structure learning.

To assess the course, 30 student evaluations from September 2020 through November 2023 were reviewed as part of a PDSA improvement cycle. Overall, MED492 was highly rated and compared favorably with other electives on standard evaluation metrics such as feedback, direct observation, teaching quality, role modeling, and teamwork. Student comments highlighted three major strengths: exposure to preoperative assessment, strong individualized teaching, and the usefulness of the topic checklist.

Areas for improvement included a need for more preoperative evaluation resources, more consistent teaching and workflow across attendings, a clearer orientation at the start of the elective, and better alignment of the rotation with student interests. Some students also noted variability in patient volume and learning opportunities.

Planned improvements include providing landmark perioperative medicine readings, creating teaching modules, introducing students to SHM and SPAQI online resources, standardizing orientation and expectations for preceptors, and involving faculty in ongoing quality improvement. Future cycles will also use quantitative data to measure progress, assess site-to-site differences, and track student comfort with preoperative evaluations before and after the elective.

Overall, MED492 offers a unique and well-received educational experience that fills an important gap in perioperative medicine training for MS4 students.
