Pdf Summary
This document summarizes a survey of adult hospital medicine fellowships in the United States and describes their current structure, goals, and outcomes.

Using the Society of Hospital Medicine website, researchers identified publicly advertised fellowships and sent a 99-field survey to program leaders. Of 40 programs contacted, 18 responded (45%). The programs were mostly based in academic medical centers, university hospitals, or teaching hospitals, and many were relatively new, with a median age of 4 years.

The survey found wide variation across fellowships, but most programs emphasized research, quality improvement, medical education, and clinical medicine. Other focus areas such as point-of-care ultrasound, healthcare administration, informatics, and global health were less common. Fellowship length averaged 1.5 years, and fellows typically spent about half their time in clinical work. Most programs offered salaries in the $60,000–$70,000 range.

About 72% of programs offered master’s-level or other postgraduate coursework, and one-third required it. Scholarly expectations often included research projects, abstracts, publications, and conference presentations. Some programs also offered certificates or distinctions in areas like POCUS, quality and safety, leadership, and medical education.

Common challenges included securing funding and FTE support, recruiting diverse fellows, balancing coursework with clinical shifts, and tailoring the curriculum to individual fellows. Reported benefits included protected time, intensive mentorship, and opportunities to develop specialized academic skills.

Overall, the study concludes that while adult hospital medicine fellowships are highly heterogeneous, they commonly provide training in research, QI, and medical education. These fellowships help hospitalists build scholarly productivity and prepare for academic careers.
