Addressing Length of Stay in Intellectual and Developmental Disability Through Individualized Care Plans

This program was designed to improve the quality of care for patients with intellectual and developmental disabilities (IDD) and reduce common barriers to effective hospitalization and discharge. Key components include transition-of-care support, where the team helps the primary care team identify discharge readiness, communicates more with caregivers, and coordinates follow-up appointments and mobile unit visits.<br /><br />The program is intended to improve several important outcomes, including length of stay (LOS), patient satisfaction, behavioral disruptions, and readmission rates. Although still in early implementation, initial hospital data are encouraging. Among program participants, average LOS decreased from 6.8 days before implementation to 5.4 days after patient-specific care plans were introduced in an acute tertiary care setting.<br /><br />The team will continue monitoring additional measures such as readmission rates, behavioral rapid response events, and patient experience scores. Early results suggest the program may help reduce hospital delays and improve care coordination for patients with IDD. Overall, the initiative offers a promising approach to addressing the complex healthcare needs and systemic barriers faced by this population.
